Any history of the following illnesses may make you ineligible for long term care insurance.

- Amyotrophic Lateral Sclerosis (Lou Gehrig’s disease)
- Asthma, emphysema, chronic obstructive pulmonary disease
- Congestive heart failure
- Cystic fibrosis
- Diabetes requiring Insulin (excluding during pregnancy)
- Huntington’s disease
- Cirrhosis of the liver
- Memory loss, dementia, confusion, organic brain syndrome
- Multiple sclerosis
- Muscular dystrophy
- Renal failure or neurogenic bladder
- Parkinson’s disease
- Peripheral vascular disease
- Post polio syndrome
- Sleep apnea
- Systemic lupus erythematosus
- Two or more (individually or in combination): Mini-stroke, Transient Ischemic Attacks, Strokes, Cerebral vascular Accidents
As well, you will be asked whether you need assistance or supervision with bathing, dressing, feeding, transferring (to or from a bed or chair), and/or toileting.
